HMC Partners used the following methods to prevent LA/SA Drug Errors

  • Tallman lettering on drugs, shelves, bins - 69%
  • Segregation of LA/SA Drugs - 39%
  • Warning stickers, colored, on LA/SA Drugs - 39%
  • Maintain & display an LA/SA List - 31%
  • Education of Clinicians - 15%
  • Limit # of available strengths of LA/SA Drugs - 8%
  • Limit # of LA/SA Drugs in automated dispensing units - 8%
  • Different color bins or shelves for LA/SA drugs - 31%
  • Warning messages in automated dispensing units, CPOE, charting systems, etc. - 46%
  • Developing a high alert medication process - 15%
  • LA/SA names are printed in color on lists & forms - 8%
  • Bar Code Technology - 8%

Responses do not total 100% because most hospital employ more than one of these measures to prevent LA/SA Drug Errors.
